We’re so pleased you’re ready to start consolidating your health records at one place, and you are ready to start managing your data. With CareXeon PHR you can manage your health information in a secure, online environment – 24 hours a day / 7 days a week. Once you create your account, you will be prompted further. Here are few steps we will be following.
- We need your consent to access your health records.
- Provide your personal information to uniquely identify you.
- We will do regional (few states) then national level search for your records.
- After finding records locations, you will be asked to choose the records you prefer imported.
- We will show the imported records and they will be stored as master records.
- From imported records, we will extract data and create PHR (EHR).
- PHR is transferable to any doctor or hospital or can be used for Tele-Health.
